If you are a referee or field marshal for Sporting Blue Valley's Recreation program, this page contains the documents required to pay you, and the rules of the game.

The form

One form, two pieces

If you referee or field marshal for Sporting Blue Valley's 2nd grade Recreation program in Fall/Spring on the weekends, you need to complete the pay form.

Already did this for Heartland? Sporting Blue Valley is separate from Heartland. We need different documentation to pay you. Even if you filled this out for them, we need you to fill it out for SBV.

Get paid

Four steps to get paid

Follow these steps exactly. If you do not complete all of them, we cannot pay you. Sporting Blue Valley will not chase you down: it is up to you to complete these steps.

  1. Complete the form

    Fill out the W-9 and DDA forms (one form, both pieces).

  2. Enter your email, and save the form to your computer

    Once you have completed the form, enter your email to save it to your computer. You need your copy to send to our bookkeeper.

  3. Email the completed form to Susan Daniels

    Send your saved copy to Susan Daniels. Susan is our bookkeeper, and the person that pays you. You can forward her the email of completion; make sure you attach the form.

    Email Susan your formsdaniels@sportingbv.org. Attach the saved form.

  4. Get paid

    Once all steps are complete, and you work your first sessions with us, you will get paid. We use a system called Melio. It takes three days to process payments. We strive to pay within a week of work, but please allow flexibility as we adjust to the new system.

Game rules

SBV Rec 2nd grade rules

  • 5 v 54 plus a goalie
  • 2 × 20minutes, 3 minute halftime
  • Size 4ball
  • Noheading, punts, PKs or direct kicks
  • 5 v 5 (4 plus a Goalie)
  • 40 minute game. Two 20 minute halves with a 3 minute halftime.
  • One center referee.
  • No direct kicks. No PKs!
  • Offside is called by the referee.
  • Goalkeepers are not allowed to punt the ball.
  • Per US Soccer Guidelines: 2nd Grade players are not allowed to head the ball.
  • No coaches on the field of play during the game.
  • Size #4 ball.
  • Opponents should be at least 10 yards back from any indirect kicks.
  • Opponents should be at least 5 yards back on throw-ins.
  • Coaches and players in the middle of the two fields with parents and other fans on the outside. No parents or coaches should be on the ends of the field (goal line).
  • Two coaches per field for a maximum of four coaches per team.
  • Coaches stay on their own half of the field during the game.
